TERMS OF REFERENCE
The Terms of Reference (ToR) outlines the scope, objectives and other essential details of the registry, and serves as a guide for all parties involved in the project, providing a clear understanding of what needs to be accomplished, the roles and responsibilities of each team member, and the overall framework for the project.

DATA ACCESS POLICY (DAP)
If you would like to participate in the inclusion of patients to the registry, please read carefully the relevant documents.
If you would like to request data from the PETER registry, please complete the DATA REQUEST FORM TEMPLATE and send the document to [email protected]. Upon receipt, the Project Management Team will send feedback within 16 weeks.
