Nurse-led Education and Support Programme for Children Undergoing Liver Transplantation: The Perspective from Coimbra Local Health Unit (Portugal)

Paediatric liver transplantation is a complex, life-changing intervention that requires comprehensive, specialised nursing care across the continuum of care. This webinar concerns to the nurse-led education and support programme for Children Undergoing Liver Transplantation at Coimbra Paediatric Hospital, highlighting the contribution of advanced nursing practice to improving patient and family outcomes throughout the transplant journey. The presentation pretend to explore a structured nursing intervention framework designed to empower children, adolescents and their families through therapeutic education, clinical surveillance and family-centred care. It addresses the pre-transplant phase, focusing on preparation for transplantation, support during the waiting period and strategies to promote treatment adherence and psychosocial readiness. The acute and long-term post-transplant phases are examined through nursing interventions aimed at early identification and prevention of complications, medication management, continuity of care and the progressive development of self-management skills according to the child’s developmental stage. Finally, the webinar addresses the transition from paediatric to adult healthcare services, although a structured nurse-led transition follow-up has not yet been established at our institution. By reflecting on this unmet need, the presentation highlights the contribution of specialised nursing to promoting continuity of care, treatment adherence, patient safety and family resilience, supporting the development of future transition pathways.
